The Daviess County Sheriff’s Office is investigating a fatal traffic accident that occurred at the intersection of State Road 257 and US 50 on the afternoon of Thursday, July 31, 2025.

According to authorities, Daviess County Central Dispatch received a call at approximately 4:41 p.m. reporting a three-vehicle crash at the intersection.

Preliminary findings indicate that 44-year-old Gina Graber of Loogootee was driving a 2014 Ford Edge eastbound on US 50, while 37-year-old Daniel Altmeyer of Huntingburg was operating a 2015 FedEx truck southbound on State Road 257. At the same time, 22-year-old Jake Kuykendall of Olney, Illinois, was riding a 2023 Honda moped northbound on State Road 257, stopped at the south side of the intersection.

A collision occurred between the Ford Edge and the FedEx truck, which then veered into the moped, striking Kuykendall. He was ejected from the moped as a result of the impact.

Emergency responders transported Kuykendall to Daviess Community Hospital. He was later transferred to a hospital in Evansville, where he tragically succumbed to his injuries during the night. His family has been notified.

The Daviess County Sheriff’s Office is leading the investigation, while the Indiana State Police are conducting a reconstruction of the accident to determine the precise cause.

Multiple agencies responded to the scene and assisted with emergency response and cleanup, including the Washington City Police Department, Washington Township Volunteer Fire Department, Indiana State Police, Daviess County Ambulance Service, Craney’s Wrecker Service, and Briggeman’s Wrecker Service.

The investigation remains active, and further details may be released as the case develops.
